Job Description

As Social Service Designee, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

Administrative Functions

  1. Assist the Program Director/Consultant in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility.

Director of Social Services:

The job position is to plan organize develop and direct the overall operation of our facility’s Social Services Department in accordance with current federal, state and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social need of the resident are met/maintain on an individual basis.

Administrative Functions

 

  1. Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.

  2. Develop preliminary and comprehensive assessments of the social service needs of each resident.

  3. Develop a written plan of care for each resident that identifies social problems/needs of the resident and the goals to be accomplished for each problem/need identified.

  4. Ensure that social services components of the MDS are completed and signed on a timely basis.

  5. Monitor the facility’s QI, QM, and survey reports relative to social services and make recommendations to correct identified problem areas.

  6. Encourage the resident/family to participate in the development and review of his/her plan of care.

JOB SUMMARY:

The primary purpose of this position is to prepare food in accordance with current applicable federal, state, and local standards, guidelines and regulations, with our established policies and procedures, and as may be directed by the Dietitian and/or Director of Food Services, to assure that quality food service is provided at all times.

MAJOR DUTIES AND RESPONSIBILITIES: include, but are not limited to

  • Review menus prior to preparation of food.
  • Assist in developing, implementing and coordinating department policies and procedures, food service manuals, job descriptions, etc.
  • Interpret department policies and procedures to personnel, residents, visitors, family members, etc., as necessary.
  • Assist in planning regular and special diet menus as prescribed by the attending physician. Assure that food is available for preparation by cooks.
  • Perform administrative requirements such as completing necessary forms, reports, etc., and submit to the Director of Food Services as required.

  • Assist in organizing, planning and directing the medical records department in accordance with established policies and procedures.
  • Assist the Medical Records/Health Information Consultant as required.
  • Maintain minutes of meetings. File as necessary.
  • Retrieve resident records (manually/electronically). Deliver as necessary.
  • Files information such as nurses’ notes, resident assessments, progress notes, laboratory reports, x-ray results, correspondence, etc., into resident charts.
  • Collect charts, assemble them in proper order, and inspect them for completion.

Education

Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
